Recognizing and Addressing Malnutrition in our Communities

Malnourished older adults are commonly discharged without a diagnosis or dietary plan. To help address community follow-through, the National Council on Aging (NCOA) surveyed several kinds of community-based organizations (CBOs) to learn what they need to bolster their ability to identify and treat malnutrition – the Community Malnutrition Resource Hub. This Resource Hub targets transitions along the clinical-community continuum of care and strives to enhance linkages across this care continuum. It provides a platform where CBOs can gain an understanding of the impact of malnutrition in older adults along with valuable, “shovel-ready” materials to use across settings, roles and responsibilities. The approach of the Community Malnutrition Resource Hub is to underscore the multidimensional nature of addressing health, well-being and, in particular, an individual’s nutritional needs.
